Cash or Debit /Credit cards / BACS transfer -WE DO NOT accept American Express or  cheques

Please bring the following with you:

·         A completed form 

·         A patient summary from your GP

·         A valid passport for ID purposes

·         Last Medical Certificate- if applicable

·         Glasses/ contact lenses if worn with an up to date spectacle prescription 

·         A Chaperone if you wish

·         A means to pay for the assessment on the day
